*Please provide names, date, and between 35 and 50 descriptive words of your choice. Please add a combination of long and short words, to achieve optimal effect. If certain words are particularly important please add a star (*) next to them in your list and I will endeavor to make these words the largest and most noticeable.

You will receive a proof via email within 1 - 2 business days of ordering. Customized items must be approved before shipping. Please respond promptly to avoid any delays in your order. Without your approval, I cannot start production on your order.
